Clomid is a prescription medication, belonging to the selective estrogen receptor modulator group of medications (SERMs) and it is primarily used in treating infertility due to anovulation in females. It works in such a way that it blocks the negative feedback on the hypothalamus, thus fooling the hypothalamus into thinking that the levels of estrogen are too low, which causes an increase in estrogen, subsequently increasing the number of ovarian follicles that are ruptured, which results in ovulation. It is one of the most commonly prescribed medications in cases of anovulation-caused infertility and its efficacy in such cases has been well documented.

Another common use for Clomid is in treating hypogonadism in males. The only reason why this use is still off-label is because no company has an incentive to pursue FDA approval for this use since the drug is generic now. It is superior to traditional testosterone replacement therapy as it does not cause sterility and e3ven enhanced the fertility. It is also much easier to use than testosterone injections or testosterone gels, as it can simply be ingested as a pill.
